Who the trip suits

On price, Russia is comparable with Turkey and Eastern Europe[2, 4]2Hip replacement cost by country — Bookimed, 20264How much does knee replacement and recovery cost abroad — MediGlobus, 2026To the list of sources →. From Western Europe, the UK and the US the trip is noticeably harder. There are no direct flights, sanctions apply and payment is difficult.

Most patients come from neighbouring countries. According to BusinesStat, Kazakhstan, Uzbekistan and Belarus bring 44 % of Russian clinics’ revenue from international patients[19]19Оборот рынка медицинского туризма в России увеличился в 2025 году на 14 % — Медвестник, 2026To the list of sources →. A Chinese-speaking interpreter is not available in every clinic, so check in advance.

Quality and risks

Periprosthetic joint infection, an infection around the implant, is the most serious complication. Russian authors put its rate after primary operations at about 3 %[6]6Эпидемиология эндопротезирования тазобедренного и коленного суставов и перипротезной инфекции в Российской Федерации — Травматология и ортопедия России, 27(3):84–93, 2021To the list of sources →. A large US study covered 1.66 million operations. How long an implant lasts depends a great deal on age and activity. Registries show that at 25 years about 82 % of knee[8]8How long does a knee replacement last? A promise of an implant for life with no mention of age is marketing.

Robotic operations in several Russian clinics are open to international patients too. Placement precision with a robot is proven; a benefit for the patient is not. Details are on the robotic joint replacement page.

Can I get a surgeon's opinion remotely before the trip?

Clinics review scans and medical reports remotely and send a preliminary treatment proposal. The surgeon makes the final decision about the operation after examining you at the clinic. Under Russian law a diagnosis cannot be made and treatment cannot be prescribed remotely.

Remote follow-up after you return is possible, within limits set by law. The unified e-visa is issued to citizens of 64 countries. The official rules list the purposes of visit as a private visit, business, tourism and events. Medical treatment is not named. Purposes outside this list require a regular visa[5]5Unified electronic visa — official FAQ — МИД России, 2026To the list of sources →. The UK, the US, Canada and Australia are not on the country list[5]5Unified electronic visa — official FAQ — МИД России, 2026To the list of sources →. Medical visa

For a longer stay you need a regular medical-treatment visa. The regional office of the Interior Ministry issues the invitation at the clinic’s request[10]10Федеральный закон № 114-ФЗ, ст. 25.6 (виза для лечения) — КонсультантПлюс, 1996To the list of sources →. Clinics ask you to allow about a month for the invitation. They request a passport copy, your address, place of work and the consulate where you will apply[3]3For foreign patients — Ilizarov Center — НМИЦ ТО им. Г. А. Visa-free entry

Even with visa-free entry, the clinic must know about your arrival in advance. Migration registration after arrival is usually done by the hotel or the clinic; check which. Insurance

The invitation and visa require a medical insurance policy valid in Russia for the whole trip. Its number and validity are entered in the invitation[1]1Russia Med Travel — official portal for international patients — ЦНИИОИЗ Минздрава России, 2026To the list of sources →. A standard travel policy does not cover a planned operation or its complications. Before admission, ask the clinic to state in writing what happens and what it costs if a repeat procedure is needed in the first weeks.

  • Vreden National Medical Research Centre, St Petersburg. A paid-services department.
  • Priorov National Medical Research Centre (CITO), Moscow. A paid-services department; robotic operations are available.
  • Ilizarov National Medical Research Centre, Kurgan. An international department with staff who speak English, French and Japanese. The centre admits more than 22,000 patients a year.   • Pirogov National Medical and Surgical Centre, Moscow. A multi-specialty centre with a commercial department. Consumables and anaesthesia are not always included in the price.

Private clinics

  • Ilyinskaya Hospital, Krasnogorsk (Moscow region). A private hospital with remote consultations and a choice of bearing surfaces.

Questions about this chapter

Can I bring a companion?

Yes. If a companion does not qualify for the clinic's invitation, they travel on their own e-visa, or visa-free if their country is on the visa-free list. Tell the clinic about the second person in advance.

Can I bring my own scans and lab tests?

Bring your MRI, CT and X-ray images on disc or as files, with the reports. Many centres do not accept lab tests from abroad, so they are repeated in Russia. This takes 1–2 days and is usually part of the programme.

What language will my discharge documents be in?

The discharge summary, implant passport and operation report are issued in Russian. Some clinics translate them into English; otherwise a translator does. Check in advance whether translation is included in the price: your doctor at home will need it.

Scans and medical reports are sent later, through a secure channel. First you give a separate consent to personal data processing. Reading the privacy notice is not consent in itself. Data is processed in Russia. For citizens of the EU and the Gulf states this is a cross-border transfer, and the consent form says so. A hospital stay lasts 3 to 10 days, depending on the clinic. Medical-tourism packages usually allow 5–7 days[2]2Hip replacement cost by country — Bookimed, 2026To the list of sources →. What moves the price within a range:

  • the implant brand and model;
  • cemented or cementless fixation;
  • the number of days on the ward and the ward category;
  • the city and the amount of rehabilitation.

What a programme includes

Russian clinics for international patients fall roughly into three tiers. These are federal centres (NMRCs, national medical research centres of the Ministry of Health), private clinics and premium JCI-accredited clinics.

JCI (Joint Commission International) is a US-based system of international hospital accreditation. In Russia only a handful of private clinics hold it. It does not apply to most state centres, and that says nothing about the quality of surgery.

The Ilizarov Centre, for example, accepts only roubles, in cash or by transfer within Russia. Transfers from abroad cannot reach it[3]3For foreign patients — Ilizarov Center — НМИЦ ТО им. Г. А. Flights

There have been no direct flights from the EU, the UK or the US since 2022. A connection in Istanbul, Belgrade, Yerevan or Dubai adds 6–12 hours to the journey. Sanctions and your own country's rules

Citizens of the EU, the UK and the US should check their own country’s restrictions. These include payments to Russian banks, consular travel advice and exclusions in insurance policies. The clinic cannot get around these rules for you, and neither can we.

Implants

Imported-brand implants are available in Russia, but supply logistics have changed. Insist that the implant brand and exact model are written into the contract and the implant passport after surgery.

Complications far from home

If a complication arises after you return, a doctor in your country will treat it, not the surgeon who operated. That is why a full set of discharge documents and an orthopaedic surgeon found at home in advance matter.

When you can fly home

There is no single rule: the surgeon clears you to fly based on how recovery is going[2]2Hip replacement cost by country — Bookimed, 2026To the list of sources →. The surgeon checks that the wound is healing and there are no signs of a blood clot. Then they issue a fit-to-fly certificate. Do not buy return tickets in advance: the trip may take longer. The main risk on the journey is deep vein thrombosis. It is a blood clot in the veins of the leg that can break off and travel to the lungs. What is usually recommended (discuss your own plan with your doctor):

  • wear compression stockings during the flight;
  • get up and walk the aisle regularly, and choose an aisle seat;
  • drink water and avoid alcohol and sleeping pills;
  • take clot-prevention medicines strictly as the surgeon prescribed, and do not stop them because of the flight;
  • flex and extend your feet while seated.
