Consent to personal data processing

The full text of the consent you give when submitting a form on the OsteoplusX website. A separate document under Russian law 152-FZ as amended by 156-FZ; data is processed in Russia.

Consent is given by you — a user of the OsteoplusX website who fills in the enquiry form. If you write on behalf of a relative, you confirm that you act with their knowledge or as their legal representative.

Consent is given to the controller (in Russian law, the «operator») — {{ORG_NAME}}, taxpayer number (INN) {{INN}}, address: {{ADDRESS}}, email info@osteoplusx.ru.

Consent is considered given when you yourself tick the box «I consent to the processing of my personal data» in the form and click the submit button. We save the date, the time of submission and a checksum of this text. This confirms which version you saw.

Consent is given freely, of your own will and in your own interest — this is the wording of the law[1]1Федеральный закон № 152-ФЗ «О персональных данных» — КонсультантПлюс, 2006To the list of sources →. In plain words: nobody forces you, and you can change your mind.

What is the data processed for?

Purposes of processing:

  1. contacting you through the channel you chose and answering your question;
  2. preparing and providing information, consulting and organisational services, if you order them;
  3. keeping a record of enquiries and storing proof of this consent.

We do not use the data for marketing mailings under this consent. We do not run mailings; if an informational newsletter is ever introduced, we will ask for a separate consent for it.

What data do you provide?

List of personal data:

  • your name, or how to address you;
  • a contact: Telegram or MAX account name, email address or phone number;
  • the joint in question;
  • the text of the message you write yourself;
  • technical data: IP address, date and time of submission, the page from which the form was sent.

Medical documents, scans and diagnoses are not covered by this consent. They are a special category of data. Please do not enter diagnoses or test results in the message field — describing your situation in your own words is enough.

What actions with the data are permitted?

You permit the following actions: collection, recording, systematisation, accumulation, storage, updating, retrieval, use, transfer to the parties named in the privacy policy, anonymisation, blocking, deletion and destruction[1]1Федеральный закон № 152-ФЗ «О персональных данных» — КонсультантПлюс, 2006To the list of sources →.

Processing is carried out both with and without automated means. The databases are located in Russia, and processing is governed by Russian law[1]1Федеральный закон № 152-ФЗ «О персональных данных» — КонсультантПлюс, 2006To the list of sources →. Data is not transferred abroad. Please note: there is no European Commission adequacy decision for Russia, so for users in the EU or the UK this means sending data to a country whose data protection rules are not recognised as equivalent to the GDPR.

Data may be shared with: a hosting provider in Russia, and a payment provider when you pay for a service. The notification sent to the manager via Telegram contains only the topic of the enquiry, the joint and the language — no name or contacts.

The consent is valid for 3 years from the date the form is submitted, or until it is withdrawn — whichever comes first[1]1Федеральный закон № 152-ФЗ «О персональных данных» — КонсультантПлюс, 2006To the list of sources →. After that, the data is deleted or anonymised unless the law requires longer storage (for example, payment records).

Write to info@osteoplusx.ru from the same email address or messenger account you used in the form. State: «I withdraw my consent to personal data processing». We will stop processing and delete the data within 10 working days. Withdrawal does not make earlier processing unlawful. If a service has already been paid for, we keep only what the law obliges us to keep.

By ticking the box, you also confirm that:

  • you are 18 or older, or you are a legal representative;
  • you have read the privacy policy;

  • you understand that the navigator’s reply is not a diagnosis or a prescription of treatment.

The second checkbox in the form: terms of service

The second checkbox is neither a data-processing consent nor a subscription. By ticking it you confirm that you have read the terms of service for the navigation services. It is not required to send an enquiry; it is required when you order a paid navigator service.

We do not run mailings. We write to the contact you provided only about your enquiry. If an informational newsletter is ever introduced, we will ask for a separate consent for it — with its own checkbox and an opt-out in every message, as the advertising law requires[3]3Федеральный закон № 38-ФЗ «О рекламе», ст. 24 — КонсультантПлюс, 2006To the list of sources →.
